In preparation for your treatment, please do not arrive on an empty stomach or come in uncomfortably full before your treatment. Being hungry may lead to feelings of faintness or dizziness. If possible, wear loose, comfortable clothing. Please be sure your sleeves and pantlegs can be comfortably pulled up over your elbows and knees, and that your abdomen is easily accessible. Do not drink alcohol or caffeine before your treatment.
Patient Forms
Patients are required to fill out forms for medical consent, intake (medical history and symptoms) and privacy (HIPAA, the Healthcare Insurance Portability and Accountability Act). To save time, please you may fill out the forms online via the link that is attached to your email confirmation.

Cancellation Policy
Kindly provide 24-hour notice for any appointment cancellation so that other patients have the opportunity for treatment if you are unable to attend your appointment. You may contact me via email, or by text or voicemail at 914- 882-4151 as soon as possible.
Confidentiality
Patient confidentiality as required by state law is maintained at all times, and I follow federal HIPAA compliance regulations.
Payment
Payment by credit card, check or cash is due upon receipt of services. If your insurance covers the treatments, I will provide you with a bill for reimbursement.
Insurance Coverage
Although I do not accept medical insurance, I will provide an insurance claim form if your policy covers acupuncture. You will be reimbursed directly through your insurance company at the rate for out-of-network services provided.
